Functional Capacity Assessment

 An Occupational Therapist (OT) performs a Functional Capacity Assessment (FCA) to evaluate a person's ability to perform essential daily tasks, such as personal care, mobility, communication, household management, and community participation. The assessment identifies areas where additional support is needed.  
After carrying out a Functional Capacity Assessment, Improve Your Move OT will develop a comprehensive report, providing evidence of the disability, its impact, and recommendations for care, therapy, equipment and assistive technology, vehicle modifications and home modifications. This report can guide the use of NDIS funding or can be used as evidence for developing an NDIS plan at the time of plan review.

Wheelchair & Seating Prescriptions

This includes assessments for manual wheelchairs, power assists, power wheelchairs, and wheelchair seating systems. Kate has experience working with complex seating systems and supporting clients with complex postural presentations. 

Additional Services include:

  • Ongoing Support: Once your wheelchair is prescribed, we offer three-monthly reviews with the Allied Health Assistant (AHA) to ensure it continues to meet your needs and remains in safe working order. For participants in Gippsland, any concerns can be escalated to Kate, who will arrange a reassessment during her next visit. Our Allied Health Assistants also help manage wheelchair servicing.
  • Wheelchair Driving Training: Through our AHA program, participants can learn to confidently drive their wheelchair. 
  • Education Sessions: We offer training for participants and their care teams on effectively using wheelchair functions in real-life settings. 
     

Complex Home Modifications

Complex home modifications are structural or extensive changes designed to improve accessibility and functionality within the home, typically costing $20,000 or more. These modifications can include:
  • Transforming bathrooms into wheelcair-accessible wet rooms.
  • Modifying kitchens and cabinetry to accomodate wheelchair use.
  • Home automation systems.
  • Ceiling hoists, elevators or stair lifts.
Under the NDIS, complex home modifications must be prescribed by a qualified home modification assessor. Kate, a certified home modification assessor, has completed NDIS-recognised training and brings extensive experience in prescribing and designing tailored home modifications to meet individual needs.

Vehicle Modifications

This service focuses on improving vehicle access for transportation. Vehicle modification options include:
  • Van Conversions with Platform Lift or Ramp: Allows wheelchair users to drive into the vehicle and be transported while safely seated in their wheelchair.
  • Wheelchair/Mobility Aid Transfer and Stowing Options: Includes boot hoists, rooftop or boot wheelchair hoists, and other systems to help stow your mobility aid.
  • Transfer Assistance Options: Automated car seats that lift you into the car, portable hoists, or sliding boards to assist with transfers into a vehicle.
  • Mobility Aid Trailer: Allows you to transport your device using the Hayman Reese of your car.
